What is Assist-Life Stage, Transition?

This is an NDIS Capacity Building support. It helps participants manage major life changes and build the skills and confidence needed to live more independently.

It focuses on:

  • Planning and preparing for life transitions

  • Building practical skills

  • Strengthening decision-making and independence

  • Coordinating supports during change
